In newly surfaced video, the 76-year-old music icon appears visibly frail, leaning heavily on a walking cane as he slowly makes his way onto a stage in Wellington, Florida. For longtime fans used to seeing Joel command arenas with swagger and confidence, the moment felt jarring — even heartbreaking.

The appearance marked Joel’s first public outing since revealing he was diagnosed in 2025 with Normal Pressure Hydrocephalus, a neurological condition that affects balance, movement, and cognition.
While his voice remained powerful as he sang Big Shot and We Didn’t Start the Fire, his body told a far more troubling story.
Joel arrived with his wife and children, and observers quickly noticed his wife staying close, appearing to help steady him as he walked. At several moments, the singer seemed cautious with every step, relying on the cane for balance as he crossed the stage.
The contrast was stark: the sound of a rock legend still in full command of his vocals — paired with the sight of a man fighting to remain upright.
Fans online described the clip as “shocking,” “sobering,” and “hard to watch.”
RadarOnline.com spoke exclusively with Dr. Kenneth Spielvogel, medical advisor at Carrara Treatment Wellness & Spa, who reviewed the footage and confirmed the signs many feared.
“What’s visible — including the use of a cane and noticeable gait instability — is consistent with balance impairment,” Spielvogel said, noting this is a hallmark symptom of Normal Pressure Hydrocephalus.
While he cautioned that a short video can’t tell the full medical story, the warning signs were clear.
“Balance issues like this are often what first alert people that something neurological is happening,” the doctor explained.
Spielvogel stressed that Joel’s future depends on how early the condition was caught, how well treatment works, and his overall health — but he acknowledged the disease is often misunderstood and frequently mistaken for more severe forms of dementia.
“This condition is not inherently terminal,” he said. “But it can significantly affect quality of life if not carefully managed.”
Radar also spoke with addiction and recovery expert Richard Taite, who weighed in on whether Joel’s long, well-documented struggles with alcohol could be complicating his condition.
“Decades of heavy drinking can absolutely weaken the brain’s resilience,” Taite said. “Alcohol impacts balance, cognition, and neurological health — especially with age.”
Still, he emphasized that Normal Pressure Hydrocephalus is a specific medical diagnosis.
“It would be wrong to say alcohol caused this,” Taite said. “But long-term sobriety gives the brain a better chance to cope.”
“What matters most now is that Billy is sober, aware, and under medical supervision,” he added. “That’s the difference between decline and stability.”
Normal Pressure Hydrocephalus is caused by an abnormal buildup of cerebrospinal fluid in the brain. Symptoms often include difficulty walking, loss of balance, cognitive changes that mimic Alzheimer’s, and bladder control issues.
The disorder typically affects adults over 60 and is sometimes referred to as “treatable dementia” because symptoms can improve with medical intervention — though recovery is far from guaranteed.
